X-linked adrenoleukodystrophy presenting with positional downbeat nystagmus.

چکیده
A 20-year-old man presented with dizziness and gait disturbance for approximately 2 months. Neurologic examination revealed spontaneous downbeat nystagmus (DBN), limb ataxia, and abnormal tandem gait (video on the Neurology® Web site at Neurology.org). Video-oculography showed augmented DBN during position change (figure 1). MRI revealed diffuse atrophy and signal changes in the brainstem and cerebellum (figure 2). The plasma levels of very long-chain fatty acids (C26:0 concentration, C24:0/C22:0 ratio, and C26:0/C22: 0 ratio) were elevated. Subsequent genetic testing revealed a missense mutation in the ABCD1 gene. The olivopontocerebellar form of X-linked adrenoleukodystrophy should be considered a rare but possible diagnosis in young men with dizziness and DBN.

The effects of baclofen and cholinergic drugs on upbeat and downbeat nystagmus.
The GABAergic drug baclofen and the cholinergic drug physostigmine were administered to patients with upbeat and downbeat nystagmus. Baclofen (orally, 5 mg three times daily) reduced nystagmus slow phase velocity and distressing oscillopsia by 25-75% in four out of five patients (two upbeat nystagmus; two downbeat nystagmus). Physostigmine (1 mg single intravenous injection) increased nystagmus...

Pearls c Eye movement abnormalities can be helpful with topical localization. c Vertical nystagmus (downbeat and upbeat nystagmus) is typically caused by posterior fossa lesions. Other causes include medication side effects, vitamin deficiencies, inflammatory and autoimmune/paraneoplastic conditions, and hereditary and degenerative cerebellar ataxias. c Lesions in the posterior fossa such as me...
متن کاملTypical and atypical benign paroxysmal positional vertigo: literature review and new theoretical considerations.
Atypical variants of benign paroxysmal positional vertigo (BPPV) are often encountered and frequently confusing. The authors present a hypothetical framework that accounts for all classical patterns of BPPV and several unusual variants such as BPPV without nystagmus, BPPV with downbeat nystagmus and BPPV with paradoxically direction-changing nystagmus. The authors introduce new concepts, such a...
متن کاملDownbeat, Positional, and Perverted Head-Shaking Nystagmus Associated with Lamotrigine Toxicity
Lamotrigine is an effective antiepileptic drug with few adverse effects. Nystagmus is one of the commonly observable signs of lamotrigine overdose, which may result from central nervous system involvement. However, the physiologic basis of lamotrigine-induced nystagmus is not fully understood. Here we report a patient who presented with lamotrigine-associated nystagmus that was probably related...
متن کاملFinger extension weakness and downbeat nystagmus motor neuron disease syndrome: A novel motor neuron disorder?
INTRODUCTION Disturbances of eye movements are infrequently encountered in motor neuron diseases (MNDs) or motor neuropathies, and there is no known syndrome that combines progressive muscle weakness with downbeat nystagmus. METHODS To describe the core clinical features of a syndrome of MND associated with downbeat nystagmus, clinical features were collected from 6 patients. RESULTS All pa...
